At Cure 4 The Kids Foundation, we believe no child should ever be denied access to life-saving care regardless of their diagnosis, insurance status, or financial situation. As Nevada’s only dedicated outpatient pediatric cancer and rare disease treatment center, we serve as an extension of the care teams who refer to us, working alongside providers to help ensure every child receives the highest quality care.
We understand that every referral represents both a clinical decision and a family searching for answers. That is why we approach every patient, family, and referring provider with empathy, urgency, and respect.
To refer a patient to Cure 4 The Kids Foundation, fax the required documents to 702-732-1080. Referrals must include patient demographics, a pediatrician or primary care provider referral, the reason for referral with suspected diagnosis, recent progress notes, and lab results from the last three months. Our team reviews each referral and contacts the family directly to schedule.

Many insurers require a referral from the patient’s pediatrician or primary care provider, and some require that referral to be submitted through the plan’s designated portal. Please verify your patient’s plan requirements before submitting. The Referral Requirements table below lists the portal and coding requirements for the plans that have them.
As a courtesy, we make every effort to contact your office if a referral is denied by insurance or if information is missing that could affect the patient’s account. We ask that the referring office follow up with the insurance company on any denial and resubmit through the appropriate channel.
Pediatric Hematology and Oncology: Leukemias, lymphomas, solid tumors, brain and central nervous system tumors, sickle cell disease, thalassemia, hemophilia, von Willebrand disease, platelet function disorders, and other bleeding and clotting conditions.
Pediatric Rheumatology: Juvenile idiopathic arthritis, lupus, vasculitis, and other autoimmune and inflammatory conditions.
Metabolic Genetics: Inherited metabolic disorders and related conditions requiring specialized genetic evaluation.
Comprehensive Clinics: Neuro-oncology, bleeding and clotting disorders, sickle cell disease, and long-term survivorship, each delivered as a single coordinated visit with the child’s full care team.
We are more than a referral destination. We are partners in care. Our multidisciplinary team brings specialized clinical expertise, innovative research, and a steadfast commitment to compassionate care to every patient and family we serve. When a child is referred to C4K, we work collaboratively with the referring provider, maintaining open communication and supporting continuity of care throughout the patient’s journey. If a service falls outside our scope, we remain committed to helping connect patients with the resources and specialists they need.
Humana, Prominence, and Select Health are contracted for Hematology, Oncology, and Metabolic Genetics. They are not currently contracted for Pediatric Rheumatology.
Select Hematology and Oncology providers are also contracted with Arizona Medicaid.

Yes. A PCP referral is one of the required documents for every referral. Many insurance plans also require the PCP to submit the referral through the plan’s designated portal, so please verify your patient’s plan requirements before submitting.

We are contracted with 22 insurance plans, including Nevada Medicaid, Medicare, Tricare, and all major commercial carriers in the state. Humana, Prominence, and Select Health are contracted for Hematology, Oncology, and Metabolic Genetics but not currently for Pediatric Rheumatology. The full list appears above.
In addition to the standard documents, these referrals require all ordered imaging, genetic testing results with the referring provider’s suspected diagnosis and clinical reasoning, and growth charts with birth history.
